Full job description Davenport-Perth Neighbourhood Centre and Community Health Centre (DPNCHC) is a multi-service organization that provides various health and community programs and services for a diverse community. We are seeking a community health ambassador (ambassador) to help increase awareness of and access to DPNCHC’s health services and community support services by conducting outreach and education activities to marginalized residents of the catchment area and to target population groups. The ambassador will also work collaboratively with a multi-disciplinary team to help plan and deliver community and wellness initiatives such as health fairs that focus on health education and screening of chronic diseases. The ambassador will also provide information on services offered by partner agencies. Responsibilities: Maintain current knowledge of the Centre’s programs and services. Engage with diverse residents and community groups in order to establish a strong community presence. Work collaboratively with a multi-disciplinary team to provide relevant, accurate and timely information related to DPNCHC’s wellness initiatives including chronic disease prevention and management, vaccine administration, mental health support, and social programs. Participate in planning and delivery of community programs and outreach initiatives that focus on health education, awareness, vaccination and screening of chronic diseases.
